After returning to the United States in 1910 in the custody of his brother, Alfred, William Chester Minor spent nine years at St. Elizabeth's hospital for the criminally insane in Washington, D.C. It was the same asylum where he had resided 42 years earlier. His delusions worsened, as did his health. In 1918, Minor was diagnosed with dementia praecox, which is now called paranoid schizophrenia.

In his last year, he lived at the Hartford Retreat for the Elderly Insane hospital in Connecticut, near the rest of his family. He was no longer considered dangerous to himself or others, and the director at the new facility allowed Minor to walk the grounds.
